Nicholas Nethercote 00e8485057 Remove all uses of DiagnosticBuilder::forget_guarantee().

There are only three. It's simpler to make the type
`DiagnosticBuilder<'_, ()>` from the start, no matter the level, than to
change the guarantee later.
