Replace cfg(dox) with cfg(doc) (#920)
`dox` has to be set explicitly, but `doc` is set whenever rustdoc runs.
This simplifies the doc process to `cargo doc` and means bootstrap can
stop passing `--cfg dox` when documenting crates.
